Establishing a positive therapeutic relationship is a vital first step in helping individuals, couples, and families. Individuals in therapy often share intimate thoughts, beliefs, and emotions. As such, it is critically important that Dr. Rosenfarb provides a safe, open, and non-judgmental atmosphere where people can feel at ease. Trust and respect are essential components of a good therapeutic bond. Dr. Rosenfarb offers individual, couple, and family sessions.
Fees
My fee is $225 per 45-minute session. I do have several sliding scale slots, although sliding scale availability cannot be guaranteed.
Insurance
I am currently contracted on the following Provider Panels:
- Medicare
- TRICARE
- TriWest
- VA Community Care Network (CCN)
- If you have a PPO insurance plan, I would be considered an out-of-network provider. PPO plans typically reimburse around 70-80% for out-of-network providers. If you are interested in using your PPO insurance, I suggest you call them and ask about their policy on out-of-network behavioral health care. You will want to know (1) the amount of your deductible and (2) what percentage of behavioral health services they will reimburse you.
I will be happy to provide you with a monthly statement that you can submit to your insurance company for reimbursement.
Good Faith Estimate Notice
You are entitled to receive a Good Faith Estimate of what the charges could be for services provided. While it is not possible to know, in advance, how many sessions may be necessary or appropriate for you, you are entitled to be provided with a good faith estimate of the cost of services. Your total cost of services will depend upon the number of sessions you attend, your individual circumstances, and the type and amount of services that are provided to you.
